Pakistan Telecommunication Authority (PTA) announced new procedure for Mobile SIM Activation guidlines from February 1st. With the help of NADRA (National Database and Registration Authority) this system has been lanuched. Massive Media (TV/Radia/News Papers) ads have been running to creat awareness among general public.
With this new system a non-activated SIM would be issued to a customer after his or her original identity card was checked and the Cellular Service Agreement (CSA) form was properly filled out.The customer would then call 789 from new SIM and the respective call centre would ask a few questions to verify the data provided.

Fill it yourself, other than mobile numbers that you want to get blocked (your respective service center will do this for you). Also leave the lower part of form.
Attach 3 copies of CNIC
And take them to concerned service center
And you are hopefully done.
1-Take some time out, before going to Service Center, as there would be plenty of other people in there for same purpose. Expect 1 hour or even one and a half hour wait before you get through
2-Please note that form downloaded from this page may not be acceptable at some service centers, as they may ask you to refill the form they will give.
3-Please make sure that you are not heading towards Franchises, because franchises donâ€™t block numbers, its only Service Center that can offer you with the service. However, those cities where service centers are not available, similar arrangements have been made by franchises to block SIMs.
Cell phone companies were not authorised to show the actual numbers under that CNIC to the complaining customer but would right away get them removed from his CNIC number. Meanwhile industry sources said that the industry in view of the good response from the subscriber to the campaign has offered to PTA that if allowed they could extend the service to select franchise outlets on nation-wide basis. “The increase in number of outlets from where service disconnection can be requested against illegally used SIMs would help us deal with the rush of customers enabling us to clean up the data quickly” the sources said adding that despite lot of rush on their CSCs due to the same, they were continuously increasing automation and manpower to deal with this important task.

Pakistan Telecommunication Authority (PTA) has advised the mobile phone subscribers to visit Customers Services Centre (CSC) of all mobile operator and get their SIM(s) data corrected against your CNIC.PTA has also requested the subscribers’ co-operation in streamlining of SIM data.
PTA has recently introduced a new service named “SIM Information System 668″ with this service the mobile subscribers to know the total number of’ SIMs issued against their names with each mobile operator by sending their CNIC number to 668. It has been observed that mobile users after getting information through this service contact the operator’s helpline service to know the details of those numbers, which have been issued against their CNIC without their knowledge. PTA has stated that the users may visit the CSC of concerned mobile operator to resolve this issue.

To purchase a new Jazz connection, visit the nearest Mobilink Customer Services Centre, Franchise Centre or Registered Retailer. Complete the Customer Services Agreement Form (CSAF) and attach along your identity documents.
Insert your new Jazz SIM in your mobile.
To activate the Jazz SIM, make a free call to 789.
Helpline 789 will require customerÃ¢â‚¬â„¢s personal details and CNIC number.
Provided antecedents will be verified by helpline through NADRA database.
Once confirmed, SIM will be activated in 30 minutes.
In accordance with PTA laws and regulations, any mobile connection which is not registered, does not carry complete information, or has false information is liable to be disconnected. Possession of such connections is considered a crime.
